CTI PY4 (FY2025) application period for new / revised CTIs opened Monday, April 8th and will close Friday, May 31st.

Care Transformation Initiatives (CTIs) are initiatives undertaken by a hospital, group of hospitals, or a collaborative partnering with a hospital to reduce the total cost of care of a defined population. Under the CTI program, hospitals choose the eligible Medicare population targeted by the CTI intervention; define how to restrict the population to those most likely to be impacted or enrolled in the intervention; and choose the intervention duration. CRISP operates the Care Transformation Profiler (CTP), to enable hospitals to view all CTIs statewide, and to monitor their own CTI progress.

Evaluation of CTI Program

AIR conducted a two-part evaluation of the Care Transformation Initiative (CTI) program. The first part was a pre-implementation report from 2021, summarizing the design and target areas of the proposed CTIs.

The second part of their evaluation reviewed findings from the first complete year of the CTI program (2021-2022). The following conclusions were discovered: (1) 32% of CTIs generated savings; (2) On average, across all CTIs, performance year costs exceeded baseline costs by more than $1 million; (3) CTIs with a focus on SDOH, performed better, on average; (4) 86% of hospitals that responded to the AIR survey reported that the implementation of their CTI was positive; (5) 75% of CTIs had fewer episodes during the performance year than at baseline; (6) The majority of interview and survey responses mentioned data access as an area for improvement; and (7) Nearly 95% of hospitals plan to continue the program in the second performance year.  A full recording of the webinar can be viewed below.
