With the launch of the Nursing Facility Connectivity Program, CRISP has partnered with Real Time Medical Systems, to provide nursing facilities (SNFs) with tools, resources, and clinical technical assistance at no cost, to further enhance patient care, data access, and facility performance.
Collaboratively, CRISP and Real Time will improve patient outcomes by reducing hospital admissions/readmissions, automating disease surveillance, establishing seamless care coordination, and improving care transitions for our states most vulnerable populations.

A recently admitted +70 year old patient with a medical history of CKD, COPD, and recent surgery had previously been inpatient at the hospital for several days. By utilizing Real Time’s Clinical Analytics Module and Clinical Technical Assistance (CTA) program, care teams were alerted on day 21 of the patient’s stay for elevated blood sugar, elevated heart rate, and low blood pressure.
This resulted in immediate action by the patients care team. Labs were collected and the patient was diagnosed with a urinary tract infection. The patient started on an antibiotic for infection and IV fluids for dehydration and vital sign support. Thanks to the care teams prompt interventions, the patient was able to be treated in place, prevent sepsis, and continue their rehabilitation from surgery without a readmission to the hospital

A medically complex patient with a history including but not limited to, CHF, ESRD (HD dep), HTN, new onset liver cirrhosis, peritonitis, DM2, and CLL, had continuously missed consult appointments due to multiple rehospitalizations. During one of the readmits to the facility, it was discovered that a specific medication was absent from the patient’s profile. Recognizing that missing a single dose of medication can start a cascade event landing the patient back in the hospital, the care team utilized Real Time’s Clinical Analytics Module and Clinical Technical Assistance (CTA) Program and were alerted through email communication to the medication oversight. This resulted in the facility making the medication correction without any delay in care and/or negative implications to the patient.
Thanks to the facility’s fast response to making changes to the plan of care, the nursing facility was able to keep the patient safe and out of the hospital – helping to reduce the number of rehospitalizations while achieving the best possible clinical outcomes.
