Social Determinants of Health (SDOH) Tools

CRISP will be demonstrating their Social Determinants of Health (SDOH) suite of tools, including the Social Needs Data tab, Screening tool, and Referral applications.

The CRISP e-Referral and Search Programs application are now live in both InContext and Portal. These applications allow users to send referrals for their patients to various community-based organizations. The search program feature allows providers to view resources available in the community for specific needs via the HIE Directory and 211’s directory.

CRISP also offers the ability for users to enter Social Needs Assessments for their patients directly in CRISP via the Accountable Health Communities- Health-Related Social Needs Screening Tool (AHC). CRISP also works with organizations to show their screening/assessment and z-code information in CRISP.

Health Equity Explorer

CRS will be demonstrating their Health Equity Explorer suite of reports, including the Disparity Index and the Social Determinants of Health Report. Health Equity Explorer is a suite of reports designed to give users an understanding of a given community’s health disparities and social determinants of health. The suite currently contains two reports: 1. Disparity Index and 2. Social Determinants of Health (SDOH).

The reports use all-payer hospital Case Mix and American Community Survey data to show users disparities in hospital utilization and social determinants by race, ethnicity, gender, condition and geography. Health Equity Explorer is now live in MD CRS.

Panel Processor

CRISP developed a more efficient way to send Encounter Notification Panels to the HIE through an application called Panel Processor. User can now leverage new features in the application to streamline sending or updating their ENS panels.

CRISP
