The Office of the National Coordinator for Health Information Technology (ONC) awarded funding to the Maryland Health Care Commission (MHCC) as part of a Challenge Grant to encourage breakthrough innovations in health information exchange, including “improving long-term and post-acute care transitions” which can be scaled and replicated across the country.
Maryland will leverage CRISP's statewide health information exchange (HIE) to share critical pieces of clinical information in real-time as residents of our state's long-term care facilities (LTCs) transition from one care setting to another.
Benefits of Participation
Access to the CRISP HIE will allow your facility’s health care providers to view hospital demographics, lab results, radiology reports, and select electronic documents such as discharge summaries, history & physicals, operative notes, and consult notes for your patients. Access to the portal is for treatment purposes only and it should not be used for admissions screening or other non-clinical purposes.
Participation in the pilot ensures free access to the CRISP HIE query tool. Participation is on a first come first served basis, and it is limited.
Timeline & Application
The enrollment period ends on February 28, 2012. The roll-out of the CRISP HIE query tool will begin in March 2012. The roll-out will be prioritized by CRISP and the timeline will be shared with all the pilot participants.
Requirements for Participation
Long-term care, post-acute such as rehab facilties and assisted living or nursing homes are all invited to participate. Internet access and Internet Explorer are required to participate in the pilot. As part of the pilot, CRISP will solicit your feedback on the HIE tool periodically over the period of the grant which ends in February 2014.
